"BIG TEN” CHRONIC DISEASE CARDS FOR AIDES NOW AVAILABLE
In 2011, the Council released its popular training publication, "The Big 10 – Keys to Keeping Consumers at Home: A Transitional Care In-service Training Curriculum for Home Care Aides.” This three-hour curriculum is designed to teach home care aides how they can best help consumers transition through the health care system and manage their chronic diseases at home.
As a companion resource for this training, our Standards Committee has developed "Big Ten” Chronic Disease Quick Reference Cards for Home Care Aides. These convenient, 5 x 7 inch laminated cards, come on a ring and can be given to aides from provide basic information, signs and symptoms, simple care tips, and red flags for working with clients with the following chronic conditions:
- Congestive Heart Failure
- Chronic Obstructive Pulmonary Disease
- Kidney Disease
- Myocardial Infarction
- Parkinson’s Disease
- Skin Care
- Urinary Tract Infection
These cards are now available for purchase from the Council. Bulk purchasing is also available. Council members can purchase up to 25 individual packets of 10 cards on a ring for $7.50 each. Orders of between 25 and 50 packets will be discounted to members only at $6.50 for each set of 10 cards on a ring. Orders of 50 or more will be discounted to members only at $5.50 per set.
